WCSC 2009 Coaches Registration and Risk Management Disclosure Form (1628)

For 2009, ALL COACHES, ASSISTANT COACHES and TRAINERS  MUST perform the following steps (except U9-U12 Competitive):

Step 1:

 

 

 

CYSA online coach registration:

  1. Read thoroughly CYSA coach registration information at link on left and follow the detailed instructions. AND
  2. Click link at bottom of above form to agree to terms  AND
  3. Select Organization: Walnut Creek SC (0402) AND
  4. Print off one copy of form (1628) and mail to registrar (Note: Competitive teams should give the form to the team manager to hand in with the team packet):

Background Check

    
WCSC Fingerprinting Background Check:
• All new and returning coaches will be fingerprinted
 
NOTE:  All information is CONFIDENTIAL and will only be securely reviewed by the League Registrar and President of the Walnut Creek Soccer Club.  This MUST be done prior to team formation.
 
 
 
 

Question: Does a coach need to get finger printed if they have already done so in another club / school, or work place?

Answer: YES.  Each agency needs to conduct their own finger printing.  The results of those finger prints are kept private within that agency.

 

Question: Why are we doing finger printing? 

Answer: By doing finger printing of all adults that are working with children we hope to offer the safest environment for the players of the Walnut Creek Soccer Club.

 

Question: Is there a cost? 

Answer: Walnut Creek Soccer Club will offer free finger printing at all recreational registration events.

 

Question: Can I get my fingerprints done by myself?

Answer: No, the club offers multiple events and they are free!!

 

Question: Can I sign up as a youth coach?

Answer: View the Youth Guidelines and download the Youth Application forms from the links below.
